ents will work in groups of 4-5 to design a QI project. The purpose of this project is to create an opportunity to practice using various QI tools and Charters in an applied project where students experience working in teams.
This project is divided into three key components. All teams will choose a Healthcare Quality Issue. Information on Healthcare Quality issues can be found as you read/review the materials in the course, book, and IHI Modules. The Groups will work on this Quality issue – developing a plan to improve the outcomes. All elements need to be completed. You will use the steps in the Charter Assignment you complete in the course – this will help with the elements for the Final Paper and Presentation. Follow the Healthcare Quality Project grading rubric.
1) Quality Improvement Project: All teams will create a Paper and Presentation. These must include an identified Healthcare quality issue – receive topic approval from instructor
· Include a diagnostic QI tool, like a flowchart or cause and effect diagram
· Include a data collection chart relevant to the project (bar chart, run chart, etc.) This chart should measure pre-post intervention.
· Consider: completeness, demonstration of critical thinking about a QI opportunity, and design aesthetics in presentation.
· Paper needs to briefly explain all elements of the presentation (transcriipt to accompany the presentation).
· The paper will explain in detail the brainstorming and aim statement
2) Presentation: Each team will give a 10-12 minute presentation about their project – voice-recorded. The presentation will clearly cover the 9 components on the Storyboard template.
a) at least one QI Tool (perhaps more)
b) at least one measure and the data collection and display process
These Healthcare Quality Project voice-recorded presentations and papers will be submitted at the end of the semester. All team members need to be responsible for a portion of the presentation. The presentation will be evaluated based on content (all 9 components included), team cohesion, and basic presentation skills. This needs to demonstrate shared work and shared presentation responsibilities.
Instructions for Storyboard/Poster:
All teams will create a Storyboard/Poster.

First your group will choose a Healthcare Quality Issue. Please have one person in your group submit your Healthcare QI issue in Blackboard for approval.
A healthcare quality improvement (QI) poster should include the following elements:
Title: Clearly reflect the project’s contents or conclusion statement
Background: Provide a brief explanation of the project’s purpose, history, and intent
Problem statement: Define the problem or scientific question that the project investigated
Aim: Include SMART aims that specify what, who, how much, and when
Interventions: Clearly state what changes were made, such as PDSA cycles
Results: Show the effects of the changes, such as PDSA data or a run chart
Conclusions: Include learning and what’s next
Tools: Highlight the QI activities and tools used in the project
Contact details: Include contact details and social media links for more information

Storyboards/Posters will be evaluated on completeness, demonstration of critical thinking about a QI opportunity, and design aesthetics. All nine required components must be addressed. The board must include at least one QI tool and an appropriate data chart.

Instructions for Presentations:
Each team will give a 10-12 minute presentation about their project. The presentation will clearly cover the 9 components on the Storyboard template. The presentation will explain in detail a) the brainstorming and aim statement writing process b) at least one QI Tool (perhaps more) c) at least one measure and the data collection and display process. These presentations need to be voice-recorded with each team member presenting on the same presentation.

All team members need to present in the presentation. All team members need to be responsible for a portion of the presentation. The presentation will be evaluated based on content (all 9 components included), team cohesion, and basic presentation skills.

The team must submit a rough outline of the plan for who will be speaking. This could take the form of an outline or slides or list of bullet points. Submit this outline in a separate document, with your presentation.

Submit the Storyboard/Poster Presentation by Sunday, December 1st@ 11:59pm.
